White Oak of Charlotte is seeking an experienced RN MDS Coordinator/Nurse Assessment Coordinator to lead MDS processes, CAAs, and care plans. You will collaborate with an interdisciplinary team to ensure regulatory compliance and accurate reimbursement while delivering person-centered resident care.

Qualifications

• Active RN license (State unspecified) required.
• Graduate of an accredited school of nursing.
• Minimum one year of long-term care experience.
• Experience as MDS Coordinator preferred.

Responsibilities

• Coordinate the Resident Assessment Instrument (RAI) processes and MDS assessments.
• DevelopCare Area Assessments (CAAs) and individualized care plans to ensure regulatory compliance and reimbursement.
• Lead care plan meetings and communicate updates with families and the interdisciplinary team.
