Job summary

The Providence Center in Rhode Island is seeking a Registered Nurse to join the Nurse ACT II Team 1B (CCBHC). The role blends office-based and community-based care, focusing on assessments, care coordination, medication administration, and client education to promote recovery and wellness.

Responsibilities include case management, care transitions, crisis response, and documentation in EMR. A valid RI RN license, driver's license, and willingness to travel within the community are required.

Qualifications

• Associate’s Degree required; Bachelor's degree preferred (Diploma in Nursing acceptable).
• Experience with psychiatric/mental health and substance use treatment preferred.
• RI licensure required: Registered Nurse.

Responsibilities

• Perform initial, annual, and ongoing nursing assessments and update care plans.
• Provide services to assigned caseload with required in-person contacts in office and community settings.
• Triage clients and conduct comprehensive health, psychiatric, substance use, housing, and safety assessments.
• Develop, implement, and coordinate individualized care plans with client goals.
• Administer medications and maintain EMR documentation; complete reconciliations.
