Job summary

Butler Hospital, part of Care New England Health System, seeks a Providence-area Registered Nurse for the CCBHC program. The role focuses on nursing assessments, care planning, medication administration, and integrated care across office- and community-based settings.

The Nurse will coordinate with multidisciplinary teams, support recovery goals, and ensure documentation meets agency standards. A Rhode Island RN license is required.

Qualifications

• Associate's degree in nursing; RN license in RI required.
• Experience with psychiatric/mental health and substance use preferred.
• Proficient in EMR documentation and medication administration standards.

Responsibilities

• Perform nursing assessments and update individualized care plans.
• Provide in-office and community-based services to a caseload.
• Triage clients for crisis, health, and safety needs.
• Administer medications and maintain EMR documentation.
• Coordinate care transitions and follow up post-discharge.
• Attend team meetings, trainings, and contribute to care planning.
