Job summary

The Providence Center Inc is hiring a Registered Nurse to assess and facilitate care for adults with co-occurring conditions, medical complexities, and unstable living arrangements. Experience with chronically homeless and addictions populations is a plus, especially within our CCBHC capacity.

The RN will perform comprehensive assessments, develop individualized care plans, coordinate care transitions, document in EMR, and educate clients and families while maintaining safety and quality

Qualifications

RI licensure as a Registered Nurse is required.
Experience with psychiatric/mental health and substance use treatment preferred.
Ability to travel within the community with reliable transportation.

Responsibilities

Perform nursing assessments and update care plans to improve health outcomes.
Provide in-person and community-based nursing services to a caseload.
Triage, crisis response, and risk management for clients.
Coordinate care transitions and post-discharge follow-up.
Document in EMR and ensure medication administration accuracy.
Educate clients and families on diagnoses, plans, and medications.
