The RN will collaborate with interdisciplinary teams, coordinate with vendor RNs, and help develop person-centered care plans to keep elders safe at home and assist with safe discharges when needed.

Qualifikationen

• MA RN license required.
• Combination of education level and related experience considered.
• Valid driver’s license and reliable transportation.
• Great assessment and verbal skills needed.
• Strong attention to detail and proficient with computer applications.

Aufgaben

• Assess and screen clients for clinical eligibility for home care services or nursing facility placement.
• Participate in developing community-based service plans with consumers and family members.
• Be an active member of interdisciplinary team meetings and coordinate with vendor RNs.
• Develop personal care plans to keep elders safe in their homes and facilitate safe discharges when appropriate.
