Qualifications

• Specialized Diploma graduate required.
• Knowledge of patient- and family-centered care and collaboration with patients and families.
• Cultural awareness and ability to work with diverse populations.
• Effective communication in transmitting and receiving information; enables partnership with patients and families.
• Knowledge of patient assessment, care planning, and discharge planning processes.
• Familiarity with health information documentation and clinical data.
• BLS certification and RN license in state of practice (PA).

Responsibilities

• Interacts with patients, families, and staff, delivering evidence-based, patient-centered care.
• Partners with patients and families to identify learning needs and provide clear education.
• Uses information systems to document care, monitor outcomes, and support quality improvement.
• Works within nursing and inter-professional teams to provide safe, compassionate care.
• Participates in safety and quality committees and implements care improvements.
• Engages in ongoing learning and acts as a preceptor where appropriate.
