Corewell Health Taylor Hospital is seeking a Nurse Navigator to assess patient needs and develop a care plan that smooths transitions from hospital to post-acute services.

You will educate patients and families, coordinate discharge planning, and collaborate with the care team to improve patient flow, safety and overall experience. On-site role with day shifts and 40 hours per week.

Qualifications

• Bachelor's in Nursing required upon hire.
• MI RN license required.
• BLS required within 90 days.
• 3+ years of acute care RN experience preferred.
• OR RN experience preferred.

Responsibilities

• Assess the patient's needs and develop a plan of care.
• Provide education and support to patients, families, staff and caregivers.
• Coordinate discharge planning for post-acute services and LOS management.
• Lead and resource the care team and promote evidence-based practice.

About The Unit

Taylor OR consists of 6 operating rooms, 1 cysto room, and 2 endo suites. Service lines include orthopedics, sports medicine, podiatry, hands, general surgery, and ophthalmology. We are comprised of RNs, surgical technologists and surgical first assistants as well as a surgical support staff to ensure we provide exceptional care for the community we serve and support surgeons and anesthesia providers throughout surgical procedures. No holidays and no Sundays. On call shifts are required.

Corewell Health Taylor Hospital is a 148-bed, Magnet®-designated hospital that is a recognized health care leader in the metro Detroit area. Proudly serving Taylor and surrounding communities since 1977, it offers specialty services that include 24‑hour emergency care, wound care and hyperbaric oxygen therapy, a surgical pavilion, a pain management clinic, orthopedic surgery, an inpatient rehabilitation facility and full‑service radiology, including advanced CT with cardiac scoring, fixed MRI including pacemaker, cinema vision and ultrasound (MSK and elastography).

Scope of work

The Nurse Navigator will assess the patient's individual needs and develop a plan of care to meet those needs. This role includes providing education and support to patients, families, staff and caregivers. The goals of this position include improving patient care and flow, eliminating barriers, and improving the overall patient experience by connecting the patient and family with appropriate resources and services. This position is designed to facilitate an effective process of the new care model; supporting quality patient care, safety and financial components; promoting integration of a seamless care model; coordinating patient discharge planning for post-acute services, length of stay (LOS) management and utilization management; collaborating to include coordination as evidenced by metrics, optimizing performance and adoption of best practice. Serves as a leader and resource for the health care team. Integrates new knowledge and evidence-based research into clinical practice. This position reports to the Clinical Nurse Manager and/or the Director of Nursing.

Qualifications

• Bachelors in Nursing Required upon hire
• MI Registered Nurse (RN) Required
• AHA or ARC Basic Life Support (BLS) Required within 90 days
• 3+ years of acute care RN experience preferred
• OR RN experience preferred
