Valley Children’s Healthcare is seeking an RN Navigator to act as an expert clinician, educator and coordinator of care. You will collaborate with providers and the healthcare team to coordinate, monitor and improve access to care and patient outcomes for families navigating complex conditions in maternal and neonatal settings.

The role requires California RN licensure, 2 years of clinical nursing experience (preferred), and a strong knowledge of nursing principles and healthcare systems.

Qualifications

• Licensed RN in CA (CA-BRN)
• 2 years of clinical nursing experience (preferred)
• Nursing degree required (2-year minimum)

Responsabilités

• Coordinate, monitor and evaluate clinical care and services for patients and families.
• Serve as a resource and primary contact to assist patients and families in navigating healthcare.
• Collaborate with Providers, Nurses and the healthcare team to improve access to care and outcomes.
• Coordinate activities across the continuum of care, interdepartmentally and across facilities.
