Job summary

CommonSpirit Oncology Mercy - Durango Medical Oncology in Durango, CO is seeking an RN Navigator to coordinate care across the continuum and promote effective resource utilization. Qualifications

• Bachelor's Degree in Nursing required.
• Three years of experience, with one year in a specialty.
• Current RN license in CO or a participating state via the National Licensure Compact.
• Current Basic Life Support (BLS) certification.

Responsibilities

• Coordinate care and services for identified patient populations across the continuum.
• Lead a multidisciplinary team to identify and remove barriers to care.
• Guide patients on treatment plans, next steps, and scheduling follow-ups.
• Educate patients and family on care guidelines and options.
• Facilitate access to program info, referrals, and counseling.
• Advocate for patients to prevent and resolve clinical issues.
• Develop relationships with physicians and providers to ensure effective communication.
