Insight Health Systems is seeking an RN Navigator to lead clinical care coordination for patients, collaborating with the care team to develop and execute comprehensive plans of care. The role emphasizes proactive needs assessment, patient education, and timely access to emotional support and guidance.

Qualified candidates hold a BSN or have at least five years of nursing experience with active RN licensure, CPR/ACLS, and a demonstrated ability to work in a multidisciplinary team to improve

Competenze

• BSN degree or 5 years of applicable nursing experience.
• Current RN license in the state of practice.
• CPR/ACLS certification.

Mansioni

• Coordinate the patient’s plan of care with caregivers and providers.
• Serve as a liaison between patients, families, providers and care locations.
• Provide education about diagnosis, treatment options, side effects, care team roles, and adherence.
• Identify and connect patients with appropriate support services and resources.
• Support care team handoffs for transitions across settings and during active treatment to survivorship or end-of-life.
