Job summary

Rocky Mountain Health Care Services is seeking a Registered Nurse in Colorado Springs to assist participants with medication management. You will assess needs, obtain Provider orders, and maintain care plans while ensuring accurate documentation in the electronic health record.

The role requires strong communication with participants and Interdisciplinary teams, reliable transportation, and proficiency with EHR systems.

Qualifications

• Graduate of an accredited Registered Nurse program and licensed as an RN in Colorado.
• Excellent written and verbal communication skills.
• Reliable transportation and valid auto insurance.
• Proficient with computer applications and Electronic Health Records.

Responsibilities

• Educate participants on best option for medication management.
• Obtain Provider orders for medication management.
• Write and maintain care plans for medication management.
• Fill medication planners safely and accurately according to orders.
• Document in electronic health records and route to Provider for co-signature.
• Communicate with participants about medications and answer questions.
