Job summary

Rocky Mountain Health Care in Colorado Springs seeks an experienced Facility Medication Nurse Coordinator. You will ensure MAR alignment between PACE and ALFs, provide up-to-date orders, and supervise medication administration with timely communication to facilities and families.

The role requires an LPN license in Colorado and at least one year of healthcare experience with frail or elderly populations; EMR proficiency and strong interdisciplinary collaboration are essential.

Qualifications

• Must be a graduate of an accredited LPN program and licensed in Colorado.
• One year healthcare experience with frail or elderly population is highly desirable.
• Proficient with computer applications, email, internet, and EMR systems.
• Knowledge of medical equipment and instruments; phlebotomy ability is a plus.
• Ability to develop and maintain department quality assurance.

Responsibilities

• Provide medication reconciliation and biweekly liaison with 20+ assisted living facilities.
• Routinely round in ALFs and interface with staff to enhance safety and satisfaction.
• Manage MAR between PACE and facilities for 200+ participants.
• Ensure accurate implementation of orders from PACE providers.
• Communicate medication/treatment instructions to staff, families, and caregivers.
• Build positive relationships with ALF leadership and staff.
• Operate with limited supervision, multitask, and prioritize tasks.
• Inform supervisor of facility issues; support medication reduction efforts.
• Track medication inventory, narcotics, and monthly Emergency Cart records.
• Maintain fridge/freezer temperature logs and EMR documentation of tests and exams.
