Genesis in Camden, ME is seeking an experienced MDS Coordinator - RN to ensure accurate MDS assessments and care plan reviews for residents. You will collaborate with nursing, dietary, social services, restorative, rehab and physician teams to meet high standards of care.

Qualifications

• Must be a graduate of an accredited School of Nursing with current Registered Nurse (RN) licensure.
• At least one year of long-term care nursing experience is required.

Responsabilités

• Manage the MDS process ensuring timely completion and review of care plans for physical, cognitive, and social factors.
• Coordinate Medicare/Medicaid case-mix documentation to optimize reimbursement for services in the nursing center.
• Integrate information from nursing, dietary, social services, and physician services to support compliant, high-quality care.
