Job summary

CommuniCare Family of Companies is recruiting a RN MDS Nurse / RAC for Wood Glen Alzheimer's Community in Dayton, OH. This full-time day shift role focuses on accurate MDS assessments and RAI coordination, with oversight of Medicare/Medicaid case mix and compliance with regulations.

The RN must be a graduate of an accredited nursing program and hold an active RN license in the state. Three years in long-term care and MDS/RAI experience are preferred.

Qualifications

• Graduate of an accredited school of nursing; RN.
• Valid RN license in the state employed.
• Three years of experience in a long term care environment preferred.
• Experience with the MDS/RAI process and/or case management preferred.

Responsibilities

• Accurate and timely completion of MDS assessments and coordination of the RAI process.
• Provides Medicare, Medicaid (case mix), and managed care oversight to ensure appropriate clinical services and reimbursement.
• Ensuring centers are in compliance with federal and state regulations and CommuniCare guidelines and policies.
• This role serves as a key member of the facility’s management team to help obtain/maintain quality outcomes.
