Job summary

CommuniCare Health in Youngstown, OH is recruiting an RN MDS Nurse to join the team. The RAC reports to the Executive Director and oversees MDS assessments and RAI coordination for Medicare/Medicaid oversight.

This role emphasizes compliance with federal/state regulations and supports quality outcomes within the CommuniCare Family of Companies. No on-call, no floor duties, and opportunity to contribute to a growing post-acute care organization.

Qualifications

• RN license in state employed.
• Three years of experience in a long-term care environment preferred.
• Experience with the MDS/RAI process and/or case management preferred.

Responsibilities

• Perform accurate and timely completion of MDS assessments and coordinate the RAI process.
• Provide Medicare, Medicaid, and managed care oversight to ensure appropriate clinical services and reimbursement.
• Ensure facility compliance with federal/state regulations and company guidelines; contribute to quality outcomes as part of the management team.
