Job summary

Wood Glen Alzheimer’s Community, part of CommuniCare Health Services, in Dayton, OH, seeks an RN MDS Nurse / RAC for a full-time day shift. No on-call and no floor nursing duties are required.

This role supports accurate MDS assessments and RAI coordination within a respected post-acute care network. The position requires an active RN license in the state, plus about 3 years in long-term care; MDS/RAI experience is preferred to ensure compliant, high-quality resident care and appropriate

Qualifications

• Graduate of an accredited school of nursing and RN license.
• Valid RN license in the state employed.
• Three years of experience in a long-term care environment preferred.
• Experience with the MDS/RAI process and/or case management preferred.

Responsibilities

• The MDS Nurse RAC coordinates accurate MDS assessments and the RAI process.
• Provides Medicare, Medicaid case mix oversight and ensures appropriate clinical services.
• Ensures compliance with federal/state regulations and company guidelines; integrates with management.
