Responsibilities

• Lead the MDS process for the SNF with accurate and timely assessments.
• Oversee Care Plans, ICD-10 coding, GG documentation, and PDPM reimbursement.
• Coordinate care plan conferences with staff, residents and families.
• Collaborate with nursing, therapy, billing and administration.
• Oversee required documentation and Physician Certifications.
• Monitor Quality Indicators and participate in QAPI initiatives.
• Provide ongoing education and guidance to staff.
