Job summary

Riverwood Health and Rehabilitation is seeking an experienced MDS Coordinator (RN) in Florida to ensure accurate MDS assessments and coordinated care plans for residents. You will work with the IDT to optimize reimbursement and compliance with state and federal guidelines.

The role requires a current Florida LPN license (RN preferred) and at least 2 years of MDS experience in long-term care. Proficiency with EHR systems like PointClickCare is highly valued.

Qualifications

• 2+ years of MDS Coordinator experience in long-term care or skilled nursing
• Knowledge of RUGs, PDPM, and Medicare/Medicaid guidelines
• Strong attention to detail and organizational skills
• Experience educating staff on MDS documentation and regulatory updates

Responsibilities

• Complete and submit MDS assessments accurately and timely
• Coordinate interdisciplinary team meetings to develop individualized care plans
• Monitor case mix index and ensure proper documentation for reimbursement
• Collaborate with nursing, therapy, and other departments for comprehensive resident assessments
• Maintain compliance with federal and state regulations including RAI and PPS
• Conduct audits and monitor quality indicators (QMs/QIs) and participate in QAPI initiatives
