Qualifications

• Current licensure as a Registered Nurse in KS or Compact Licensure.
• Minimum 2 years of experience in gerontological nursing or a related field.
• Strong clinical skills and knowledge of gerontological nursing practices.

Responsibilities

• Conduct initial and comprehensive resident assessments using the MDS and develop individualized care plans.
• Lead and participate in care plan conferences to ensure comprehensive care.
• Collaborate with the interdisciplinary team to develop and implement resident care plans.
• Identify and prioritize resident needs, and coordinate care to meet those needs.
• Maintain accurate and timely records of resident assessments, care plans, and progress.
• Participate in quality improvement initiatives to enhance resident outcomes and care.
