Qualifications

• Current RN licensure in Kansas or Compact Licensure.
• Minimum 2 years of gerontological nursing experience.
• Strong communication, organization, and interpersonal skills.

• Experience with MDS processes is preferred.

Responsibilities

• Conduct initial and comprehensive resident assessments using the MDS and develop individualized care plans.
• Lead and participate in care plan conferences to ensure comprehensive care.
• Collaborate with the interdisciplinary team to develop and implement resident care plans.
• Identify and prioritize resident needs, and coordinate care accordingly.
• Maintain accurate records of assessments, care plans, and progress.
• Participate in quality improvement initiatives to enhance resident outcomes.
