Responsibilities

• Coordinate and monitor timely completion and submission of all MDS assessments, ensuring deadlines are met.
• Collaborate with nursing and therapy leadership to ensure accurate PDPM coding and capture of clinical conditions and needs.
• Track Medicare/Medicaid assessment schedules and triggers to prevent missed or late assessments.
• Lead and participate in care plan meetings with measurable outcomes and updated goals.
