Job summary

Genesis Healthcare is seeking an MDS Coordinator - RN to ensure timely, accurate MDS assessments and care plan reviews in our long‑term care setting. You will coordinate Medicare/Medicaid case-mix documentation to support appropriate reimbursement, collaborating with nursing, dietary, social services and physicians.

The ideal candidate is an RN with long-term care experience, with LPNs with MDS exposure considered. At least one year in long-term care nursing is required.

Qualifications

• Must be a graduate of an accredited School of Nursing with current Registered Nurse (RN) licensure.
• LPNs with MDS experience may be considered.
• At least one year of long-term care nursing experience is required.

Responsibilities

• Manage the MDS coordination process and timely completion of the MDS assessment tool.
• Review care plans and their delivery for physical, cognitive, and social factors to ensure high standards of care.
• Coordinate Medicare/Medicaid case-mix documents to assure appropriate reimbursement.
• Conduct concurrent MDS reviews to achieve maximum allowable RUG categories.
• Integrate information from nursing, dietary, social services, restorative, rehabilitation, and physician services to support accurate reimbursement.
