Job summary

SNF Staffing Solutions seeks a skilled MDS Coordinator to ensure timely and accurate completion of the RAI and care management processes from admission to discharge in line with company policy and federal/state guidelines.

Under the direction of the Director of Nursing and VP of Clinical Reimbursement, you may coordinate information systems operations and education for the clinical department. Relocation to New Mexico is required.

Qualifications

• Graduate of an approved Registered Nurse program and licensed in the state of practice.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.
• Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement.
• Experience with MDS 3.0, preferred.

Responsibilities

• Works with the Interdisciplinary Team to assess resident needs and schedule MDS assessments and care plan reviews as required by governing agencies.
• Ensures decisions on completing or not completing additional MDS assessments are based on clinical criteria per the RAI User’s Manual.
• Assist with coordination and management of daily stand up meetings, review resident care, and set assessment reference dates.
• Complies with federal and state regulations regarding completion and coordination of the RAI process.
• Monitors MDS and care plan documentation and ensures it's present in the medical record to support MDS coding.
• Maintains current MDS status according to state and federal guidelines.
• Maintains frequent and accurate data entry into MDS programs.
• Completes accurate coding of the MDS from medical records and interviews.
• Attends interdisciplinary team meetings and other meetings to gather information and communicate changes.
• Assists DON with identification of significant changes and physician orders to reflect changes in MDS and care plans.
• Prepares scheduling and notices for care planning conferences and communicates outcomes to staff and residents.
• Keeps knowledge base updated on data entry and computer technology.
• Completes electronic submission of required documentation to the state database and other entities.
• Ensures final MDS is completed and submitted to state and federal agencies.
• Instructs staff in the RAI process, PPS Medicare, and clinical computer systems.
• Maintains confidentiality of necessary information.
• Other duties may be assigned as needed.
