Job summary

Cordova Health & Rehabilitation is seeking an experienced MDS Coordinator (RN) to join our nursing leadership. In this role you will coordinate MDS assessments, ensure regulatory compliance, and lead care-planning efforts with an interdisciplinary team.

You will ensure accurate, timely resident assessments and oversee development of individualized plans, while supporting quality outcomes and survey readiness in a compassionate, team-focused environment.

Qualifications

• Active Alabama RN license in good standing.
• Prior MDS experience in Long-Term Care or Skilled Nursing preferred.
• Knowledge of MDS, RAI, care planning requirements.
• Familiarity with CMS regulations governing long-term care.
• Strong clinical assessment and documentation skills.
• Excellent communication with interdisciplinary teams.

Responsibilities

• Coordinate and complete MDS assessments accurately and on time.
• Ensure compliance with RAI, CMS, and state/federal regulations.
• Develop and update individualized resident care plans.
• Lead interdisciplinary MDS care plan meetings and document outcomes.
• Monitor schedules, deadlines, and documentation accuracy.
• Collaborate with nursing, therapy, social services, and dietary teams.
• Review clinical documentation for accuracy and regulatory compliance.
• Support QAPI initiatives and survey readiness.
