Fair Oaks Health and Rehab in Fairfax, VA is seeking an experienced MDS Coordinator (RN) to oversee resident assessments and care planning. You will collaborate across departments to ensure compliant documentation and maximize appropriate reimbursement while maintaining high-quality resident care.

The role requires an active RN license in Virginia and MDS/RAI proficiency, with strong attention to detail and teamwork.

Qualifications

• Active RN license in VA.
• MDS experience (1+ year preferred).
• Strong knowledge of PDPM, RAI guidelines, and care plan development.
• Excellent attention to detail and organizational skills.
• Ability to work collaboratively with nursing, rehab, and administrative teams.
• Certification in MDS/RAI preferred, or willingness to obtain.

Responsabilités

• Complete and manage timely, accurate MDS assessments (per RAI process & CMS guidelines).
• Coordinate interdisciplinary care planning meetings.
• Review documentation to support coding accuracy for PDPM.
• Monitor quality measures and support QA initiatives.
• Educate staff on best practices in documentation and resident care planning.
• Stay current with regulatory updates and ensure compliance.
