Job summary

Lakeside Health & Rehab in Richmond, VA is seeking an experienced RN MDS Coordinator to join our clinical leadership team. This role focuses on accurate MDS assessments, timely documentation, and interdisciplinary collaboration to support high-quality resident care.

You will oversee the MDS process, ensure CMS/RAI compliance, drive PDPM reimbursement, and mentor nursing staff across departments, with a commitment to survey readiness and ongoing quality improvement.

Qualifications

Active RN license in Virginia is required.
Experience with MDS assessments and RAI guidance preferred.
Strong documentation and regulatory knowledge for CMS requirements.

Responsibilities

Complete accurate MDS assessments in line with RAI/CMS guidelines.
Coordinate the MDS process and meet required timeframes.
Lead interdisciplinary care planning and review documentation for coding and PDPM reimbursement.
Collaborate with nursing, therapy, social services, dietary, and others.
Monitor quality measures and support quality improvement initiatives.
Identify documentation opportunities and educate nursing staff on best practices.
Ensure care plans reflect resident needs and stay current with regulations.
