Otterbein SeniorLife is seeking an experienced MDS Coordinator (RN) for our Marblehead, OH community. You will coordinate accurate MDS assessments, develop person-centered care plans in collaboration with the interdisciplinary team, and ensure compliant documentation and coding to support reimbursement.

The role requires an active Ohio RN license and at least three years of MDS experience in a long-term care setting.

Qualifications

• RN license and good standing with Ohio Board of Nursing required.
• Three years of MDS experience in long-term care or nursing home preferred.

Responsibilities

• Coordinate and complete accurate, timely MDS assessments.
• Create and update individualized, person-centered care plans.
• Review medical records to support MDS coding and care planning.
• Ensure documentation supports assessment findings and reimbursement.
• Coordinate data entry, submission, and transmission within deadlines.
