Job summary

Life Enriching Communities in Reynoldsburg, OH, seeks a Clinical MDS and Care Coordination Manager to lead MDS assessments, care planning, and interdisciplinary collaboration within our skilled nursing unit.

You will ensure regulatory compliance, coordinate transitions, and support residents and families while guiding the care team to deliver compassionate, high-quality outcomes. This role includes on-call coverage and on-site facility leadership.

Qualifications

• Valid Ohio RN or LPN license.
• 2 years of MDS experience or in Case Management/Care Transitions.
• Leadership/healthcare leadership experience in SNF or hospital setting.
• Customer service orientation with strong problem-solving.
• Strong teamwork and ability to work under pressure.
• Valid state driver’s license.
• Demonstrated familiarity with healthcare technology and data analytics.

Responsibilities

• Organize and schedule care conferences with patients and families at admission and quarterly; coordinate community resources and assess risk.
• Consult with physicians and care team to address social, emotional, and environmental factors affecting health.
• Oversee discharge planning for short-term/skilled residents.
• Participate in interdisciplinary care conferences.
• Manage documentation for Part A and Part B billing, including notices.
• Develop and maintain MDS schedules; ensure timely submission of data.
• Coordinate resources to optimize each patient’s care plan.
• On-call duties may include weekends and evenings.
• Review referrals for the Health Center submitted by Admissions Coordinator.
