Job summary

FellowshipLIFE Inc in Manchester Township, NJ is seeking an experienced MDS Coordinator RN to join our care team. This full-time role focuses on coordinating, completing, and transmitting MDS assessments in line with CMS and PDPM guidelines, using PointClickCare for documentation and care planning.

The role requires an active RN license and at least two years of MDS coordination experience in SNF or LTC settings, with strong knowledge of CMS regulations and regulatory compliance.

Qualifications

• Active RN license required.
• Minimum 2 years of MDS Coordinator experience in SNF/LTC.
• Experience with MDS 3.0, PDPM, Medicare/Medicaid.
• Proficiency with PCC and EMR systems.
• Strong knowledge of CMS regulations and compliance requirements.

Responsibilities

• Coordinate, complete, and transmit MDS assessments per CMS/state/federal regulations.
• Ensure timely completion of MDS for Sub-Acute, Medicare, Managed Care, and LTC residents.
• Demonstrate knowledge of PDPM and its impact on reimbursement and care planning.
• Utilize PCC for MDS documentation, assessment management, and care planning.
• Collaborate with nursing, therapy, dietary, social services, and other teams on assessments and documentation.
• Conduct resident reviews and chart audits for accuracy and coding compliance.
• Lead Triple Check Process to ensure billing accuracy and regulatory compliance.
• Develop and oversee comprehensive individualized care plans based on assessments.
