You will lead a collaborative team, review clinical data for accuracy, and oversee regulatory reporting. RN licensure in NJ and LTC experience are required; BSN preferred. This is a full-time on-site role with competitive benefits.

Qualifications

• Experience coordinating MDS assessments and care plans.
• Knowledge of Medicare/Medicaid requirements and timelines.
• Strong understanding of LTC regulations and documentation.

Responsibilities

• Coordinate interdisciplinary team to complete MDS assessments and care plans per schedule.
• Oversee MDS data transmission to state and federal agencies with accuracy and timeliness.
• Lead daily admission and care planning meetings; communicate acuity changes to leadership.
• Audit medical records to ensure documentation supports MDS coding and billing.
