Holy Family Home is seeking an experienced RN MDS Coordinator or RNAC to join our 18-bed skilled nursing facility in Philadelphia. You will oversee the MDS process, care planning, and inter-disciplinary collaboration to optimize resident well-being and regulatory compliance.

The role requires an active RN license, up-to-date vaccination, and strong coordination skills to manage assessments, care conferences, and state reporting. You will work within a supportive, family-centered environment.

Qualifications

• Must hold an active RN license and be up to date with state requirements.
• Experience coordinating MDS assessments in long-term care.
• Ability to schedule and document care conferences and care plans.

Responsibilities

• Oversee initiation and completion of comprehensive MDS assessment and care plan to ensure services meet residents’ needs.
• Schedule and participate in weekly care conferences; ensure documentation reflects MDS reviews.
• Coordinate Resident assessments including MDS, CAAS, quarterly/annual reviews; coordinate signatures.
• Transmit weekly updates to state and maintain 802 reports.
• Maintain resident confidentiality and uphold resident rights.
• Meet daily with therapy manager to monitor Medicare Part A RUG classifications and therapy admissions/discharges.
• Participate in UMR reviews and State surveys.
• Initiate notification letters for therapy start of care and 48-hour notices when needed.
