Qualified applicants are RN-licensed in Pennsylvania, with a 2-year degree preferred and RAC certification preferred. Prior long-term care experience and knowledge of PPS/PDPM are highly valued.

Qualifications

Responsibilities

• Complete and coordinate the RAI/MDS assessment process for assigned residents, ensuring accurate documentation and timely submissions.
• Develop comprehensive resident care plans at admission, annually, and after significant changes to optimize outcomes.
• Coordinate with the IDT, schedule assessments, and lead MDS-related meetings as needed.
• Monitor clinical documentation for accuracy, regulatory compliance, and proper coding, including 7-day look-back.
• Support reimbursement and case-mix determination through correct MDS coding and PDPM data.
• Maintain up-to-date knowledge of MDS/RAI requirements and prepare information for insurers.

Pay: Up to $41/hour

Holy Family Senior Living is seeking compassionate and dependable RN MDS Coordinator (RNAC) to join our team in Bethlehem, PA. At Holy Family, we care like family.

What You'll Do as an RN MDS Coordinator (RNAC):

• Complete and coordinate the RAI/MDS assessment process for assigned residents, ensuring assessments, MDS documentation, CAAs, and related information are accurate, complete, and submitted within required timeframes.
• Develop comprehensive, individualized resident care plans upon admission, annually, and following significant changes, with a focus on achieving the best possible resident outcomes.
• Coordinate with the interdisciplinary team (IDT) by conducting resident and staff interviews, communicating assessment schedules, monitoring required documentation, and leading care plan or MDS-related meetings as assigned.
• Monitor and audit clinical documentation to ensure accuracy, timeliness, regulatory compliance, and proper coding, including required nursing documentation and 7-day look-back information.
• Support accurate reimbursement and case-mix determination by ensuring appropriate MDS coding, diagnoses, ancillary usage, and Section GG information used to determine PDPM and other applicable reimbursement levels.
• Maintain current knowledge of MDS/RAI requirements and regulations, participate in ongoing education, and prepare resident status updates and other required information for insurance providers.

RN MDS Coordinator (RNAC) Qualifications:

• Registered Nurse (RN) eligible for licensure in the Commonwealth of Pennsylvania
• 2-year college degree preferred
• Resident Assessment Coordinator certification preferred
• At least 1 year of experience in long-term care
• Knowledge of the MDS and Care Plan process
• Knowledge of PPS, State Case Mix, and PDPM
• Strong verbal and written communication skills
