Job summary

The Gardens at Easton, a skilled nursing community in Easton, PA, is seeking an experienced RN Assessment Coordinator (MDS Coordinator) to lead the resident assessment process and ensure timely, accurate completion in compliance with state and federal regulations.

The successful candidate will manage MDS scheduling, coordinate CAAs and care plans with interdisciplinary teams, and transmit data to the state weekly.

Qualifications

• Current Registered Nurse (RN) license in Pennsylvania.
• Minimum of one year of MDS Assessment Coordinator experience in a long-term care setting.
• Strong organizational, assessment, documentation, and communication skills.
• Knowledge of state and federal long-term care regulations.

Responsibilities

• Perform resident assessments routinely and as needed for condition changes, hospital returns, or unscheduled events.
• Accurately observe, assess, and report changes in resident conditions to appropriate personnel.
• Ensure timely completion of assessments and Care Area Assessments (CAAs) by interdisciplinary departments, including Social Services, Activities, and Dietary.
• Complete MDS assessments, quarterly reviews, CAAs, and related care plans.
• Initiate, update, and maintain care plans based on resident needs identified through assessment data.
• Prepare and distribute MDS schedules to interdisciplinary team members.
• Transmit MDS data to the state weekly, or as required, and maintain accurate transmittal documentation and logs.
