Forest City Nursing and Rehab in Forest City, PA is seeking an experienced Registered Nurse Assessment Coordinator (RN MDS Coordinator) to lead the resident assessment process. This on-site role ensures timely, compliant MDS activities across interdisciplinary teams.

The ideal candidate has a PA RN license, extensive LTC MDS experience, and at least 1 year as an MDS Coordinator, with strong regulatory knowledge and organizational skills.

Qualifications

• Current Registered Nurse license in Pennsylvania.
• Extensive MDS experience in a long-term care/skilled nursing setting.
• Minimum 1 year of MDS Assessment Coordinator experience in a long-term care setting.
• Strong knowledge of state and federal regulations related to MDS assessments and resident care planning.
• Excellent organizational, communication, and time-management skills.

Responsibilities

• Perform resident assessments routinely and for condition changes, hospital returns, or unscheduled events.
• Observe, assess, and report changes in resident conditions to appropriate personnel.
• Ensure timely completion of assessments and CAAs by interdisciplinary departments, including Social Services, Activities, and Dietary.
• Complete MDS assessments, quarterly reviews, CAAs, and related care plans.
• Initiate, update, and maintain care plans based on assessment data.
• Prepare and distribute MDS schedules to interdisciplinary team members.
• Transmit MDS data to the state weekly or as required and maintain accurate transmittal documentation and logs.
