Fairlane Gardens Nursing and Rehab in Reading, PA is seeking an experienced Registered Nurse Assessment Coordinator (RN MDS Coordinator) to lead the resident assessment process and ensure timely, compliant completion of MDS, CAAs, and care plans. You will coordinate interdisciplinary teams and maintain transmittal documentation.

The role emphasizes accuracy, regulatory knowledge, and leadership in the assessment program within a skilled nursing setting.

Qualifications

• Current RN license in Pennsylvania.
• Minimum 1 year of MDS Assessment Coordinator experience in a long-term care setting.
• Strong organizational, assessment, documentation, and communication skills.
• Knowledge of state and federal long-term care regulations.

Responsibilities

• Perform resident assessments routinely and as needed for condition changes, hospital returns, or unscheduled events.
• Accurately observe, assess, and report changes in resident conditions to appropriate personnel.
• Ensure timely completion of assessments and CAAs by interdisciplinary departments, including Social Services, Activities, and Dietary.
• Complete MDS assessments, quarterly reviews, CAAs, and related care plans.
• Initiate, update, and maintain care plans based on resident needs identified through assessment data.
• Prepare and distribute MDS schedules to interdisciplinary team members.
• Transmit MDS data to the state weekly, or as required, and maintain accurate transmittal documentation and logs.
