Myerstown Nursing and Rehab in Myerstown, PA is seeking an experienced Registered Nurse Assessment Coordinator (RN MDS Coordinator) to lead the resident assessment process and ensure timely, accurate documentation in compliance with state and federal regulations.

The role requires an active PA RN license and a minimum of 1 year of MDS experience in a long-term care setting. This is a full-time position with a competitive salary commensurate with experience.

Qualificações

• Current Registered Nurse (RN) license in the state of Pennsylvania.
• Minimum 1 year of MDS Assessment Coordinator experience in a long-term care setting.

Responsabilidades

• Perform resident assessments routinely and as needed for condition changes, hospital returns, or unscheduled events.
• Accurately observe, assess, and report changes in resident conditions to appropriate personnel.
• Ensure timely completion of assessments and CAA’s by interdisciplinary departments, including Social Services, Activities, and Dietary.
• Complete MDS assessments, quarterly reviews, CAA’s, and related care plans.
• Initiate, update, and maintain care plans based on resident needs identified through assessment data.
• Prepare and distribute MDS schedules to interdisciplinary team members.
• Transmit MDS data to the state weekly (or as required) and maintain accurate transmittal documentation and logs.
