The Gardens at Easton, PA is seeking an experienced Registered Nurse Assessment Coordinator (RN MDS Coordinator) to lead the resident assessment process in our skilled nursing unit in Easton, PA. This role ensures timely and accurate MDS assessments, CAAs, and care plans in compliance with state and federal regulations.

You will collaborate with interdisciplinary teams, schedule MDS timelines, and maintain documentation and transmittal logs.

Qualifications

• Current RN license in Pennsylvania.
• Minimum of one year MDS Assessment Coordinator experience in a long-term care setting.
• Strong organizational, assessment, documentation, and communication skills.
• Knowledge of state and federal long-term care regulations.

Responsibilities

• Perform resident assessments routinely and as needed for condition changes, hospital returns, or unscheduled events.
• Accurately observe, assess, and report changes in resident conditions to appropriate personnel.
• Ensure timely completion of assessments and Care Area Assessments (CAAs) by interdisciplinary departments, including Social Services, Activities, and Dietary.
• Complete MDS assessments, quarterly reviews, CAAs, and related care plans.
• Initiate, update, and maintain care plans based on resident needs identified through assessment data.
• Prepare and distribute MDS schedules to interdisciplinary team members.
• Transmit MDS data to the state weekly, or as required, and maintain accurate transmittal documentation and logs.

The Gardens at Easton is seeking an experienced and detail-oriented Registered Nurse Assessment Coordinator (RN MDS Coordinator) to join our skilled nursing team in Easton, PA. In this essential role, you will lead and oversee the resident assessment process, ensuring timely and accurate completion in compliance with all applicable state and federal regulations.
