Qualifications

• Current RN license in the state of Pennsylvania.
• Minimum 1 year of MDS Assessment Coordinator experience in a long-term care setting.
• Detail-oriented with strong documentation skills.

Responsibilities

• Perform resident assessments for condition changes, hospital returns, or unscheduled events.
• Observe, assess, and report changes in resident conditions to appropriate personnel.
• Ensure timely completion of assessments and related care plans by interdisciplinary teams.
• Complete MDS assessments, quarterly reviews, CAA’s, and related care plans.
• Initiate, update, and maintain care plans based on assessment data.
• Prepare and distribute MDS schedules to interdisciplinary team members.
• Transmit MDS data to the state weekly (or as required) and maintain logs.
