Job summary

PACS in California is seeking an experienced LPN/LVN Shift Supervisor to lead the nursing team in a long-term care setting. You will oversee assessments, care plans, admissions, discharges, and daily nursing operations to meet regulatory standards.

You will direct nursing staff, ensure proper documentation, medication administration, and staff development, while maintaining a warm, patient-centered environment consistent with facility policies.

Qualifications

• Must possess a Nursing Degree and active LPN/LVN license in the state.
• Strong leadership and supervisory experience in a nursing setting.
• Knowledge of regulatory requirements for long-term care facilities.
• Experience with care planning, patient assessments, and documentation.

Responsibilities

• Lead nursing service on your shift and supervise CNAs and nursing staff.
• Administer and supervise medication administration as ordered by physicians.
• Develop, review, and implement resident care plans and discharge plans.
• Ensure accurate charting, reports, and adherence to policies and procedures.
• Coordinate with other departments to ensure seamless resident care.
