Qualifications

Responsibilities

• Coordinate completion and submission of MDS Assessments, CCAs, and Care Plans.
• Report changes in patient condition identified by MDS to the DON.
• Educate direct care staff on CMS RAI updates affecting documentation.
• Review and assist with Interdisciplinary Comprehensive Care Plans.
