Five Oaks Rehabilitation and Care Center is seeking an experienced MDS Coordinator RN to lead comprehensive resident assessments and coordinate care plans with an interdisciplinary team in North Carolina. The role emphasizes regulatory compliance andStaff education to ensure optimal outcomes.

Qualifications

• Active RN license required.
• Proven experience in clinical assessment and MDS coordination.
• Strong knowledge of federal and state regulations related to MDS and care planning.
• Exceptional organizational skills and attention to detail.
• Excellent communication and interpersonal skills.

Responsibilities

• Conduct and coordinate comprehensive assessments of new and existing residents using the MDS tool.
• Develop, implement, and review individualized care plans in collaboration with an interdisciplinary team.
• Ensure compliance with federal and state regulations for MDS assessments and care planning.
• Monitor and transmit MDS assessments to the relevant authorities.
• Train and guide nursing staff on MDS procedures and updates.
• Audit clinical records to ensure accuracy, completeness, and regulatory compliance.
• Collaborate with healthcare providers to optimize resident care outcomes.
