Job summary

White Oak of Shelby is seeking an experienced Registered Nurse to join as an MDS Coordinator. You will coordinate RAI, MDS assessments, CAAs, and care plans with an interdisciplinary team to ensure regulatory compliance and proper reimbursement.

This role emphasizes accuracy, leadership, and resident advocacy in a long-term care setting. Ideal candidates hold an active RN license, have long-term care exposure, and can lead care plan meetings while ensuring high-quality, person-centered care for

Qualifications

• Active RN license in good standing.
• Graduation from an accredited school of nursing.
• Minimum one year of long-term care experience; MDS Coordinator experience preferred.

Responsibilities

• Coordinate the Resident Assessment Instrument (RAI) process and MDS assessments.
• Develop CAAs and individualized care plans within regulatory timelines.
• Ensure regulatory compliance and accurate reimbursement through documentation.
• Lead care plan meetings with families and interdisciplinary team.
• Manage Medicare PPS, PDPM, and Managed Care schedules.
• Educate staff on documentation and resident assessment processes.
