Job summary

Majestic Care is seeking an experienced MDS Coordinator (RN) to oversee MDS and care plans for residents in our West Virginia facility. You will supervise and coordinate all MDS and care plans, ensuring timely documentation and compliance with state requirements.

The role requires coordinating PPS and MDS-related assessments, collaborating with the care team, and maintaining up-to-date knowledge of relevant laws and guidelines.

Qualifications

• Must hold current RN or LPN license in the state of employment.
• Experience in Long Term Care preferred.
• Proficient with MDS 3.0 and care-planning processes.
• Strong data entry and word processing skills.
• Knowledge of state/federal guidelines for nursing facilities.
• CPR certification or ability to obtain within 30 days.

Responsibilities

• Coordinate MDS and Care Plans in a timely, compliant manner.
• Inform care team of due care plans and ensure team contributions.
• Develop or revise resident care plans quarterly or with changes.
• Prepare PPS and required MDS assessments; ensure CATs and CAAs completed.
• Coordinate care plan meetings and encourage team participation.
• Ensure timely submission of MDS to state/federal agencies.
• Maintain resident rights and confidentiality in MDS processes.
• Communicate across departments to update MDS and care plans.
• Monitor care during MDS process and report concerns.
• Assist with pre-admission screening for potential RUGs.
