Aventura at the Bay is seeking an RN-qualified MDS Nurse Coordinator to manage resident assessments and MDS submissions for regulatory compliance. You will support the IDT in care planning and ensure accurate coding across the facility.

Responsibilities include leading MDS processes, CAAs, and care-plan development, while collaborating with nursing, therapy, dietary, and social services. Prioritize accuracy, HIPAA, and team communication.

Qualifikationen

• RN license in good standing is required.
• Minimum of 2 years of MDS 3.0 experience in a long-term care setting.
• Strong knowledge of RAI process and Care Plan development.
• Familiarity with Medicare/Medicaid reimbursement processes and PDPM.

Aufgaben

• Manage and complete MDS assessments per CMS and state rules.
• Ensure timely submission and coding accuracy of MDS.
• Oversee CAAs and support individual care plans.
• Lead interdisciplinary care plan meetings.
• Monitor nursing documentation for MDS coding accuracy.
• Coordinate data collection with nursing, therapy, dietary and social services.
• Track assessment schedules to stay compliant.
• Support quality improvement initiatives and audits.
• Provide MDS training to facility staff.
