Job summary

Aventura Health Group is seeking a Nurse Assessment Coordinator (MDS/RNAC) at Prospect Park to coordinate resident assessments and ensure compliant documentation that upholds quality care.

Qualifications

• Active RN or LPN license in the applicable state.
• At least 1 year of MDS/RNAC experience in skilled nursing or long-term care.
• Strong knowledge of MDS 3.0 and CMS regulations.
• Experience with care planning and interdisciplinary coordination.

Responsibilities

• Coordinate and complete the MDS assessment process per regulations.
• Ensure timely, accurate submissions of MDS assessments.
• Collaborate with nursing, therapy, dietary, social services, and others.
• Participate in interdisciplinary care planning meetings.
• Monitor MDS schedules and supporting documentation.
• Maintain HIPAA confidentiality and compliance.
• Provide staff education on MDS processes.
