Job summary

Little Falls Health Services in Little Falls, MN is seeking an experienced MDS Coordinator (RN) to manage the Resident Assessment Instrument (RAI) process, including the MDS, Care Area Assessments, and care plans, ensuring regulatory compliance and quality resident outcomes.

Qualifications

• Prior MDS/RAI experience preferred (will train the right candidate)
• Knowledge of state and federal long-term care regulations
• Strong assessment, communication, and documentation skills

Responsibilities

• Plan, schedule, and complete MDS assessments and submissions
• Lead interdisciplinary care planning and documentation review
• Identify and address changes in resident conditions
• Monitor Medicare eligibility and documentation
• Ensure compliance with state and federal guidelines
• Support Quality Assurance and Performance Improvement (QAPI) efforts
• Educate and support clinical staff on the MDS process
