Responsibilities

• Conduct assessments and interviews for new residents to develop personalized care plans.
• Design and implement care plans with measurable goals for resident progress.
• Facilitate interdisciplinary meetings with healthcare professionals, families, and caregivers.
• Partner with specialists to optimize resident outcomes.
• Lead in-service training on MDS, RAPS, and Triggers to keep staff informed.
• Manage the MDS/Care Planning schedule for non-Medicare residents and meet deadlines.
