Job summary

The Grove of La Grange Park is seeking an RN MDS Coordinator to join our caring team. You will develop, monitor and audit resident care plans, conduct assessments, and assist with discharge planning to ensure high-quality, person-centered care in a post-acute setting.

Ideal candidates will hold an Illinois RN license and have completed an accredited nursing program; prior MDS coordination experience is preferred and CNA experience is a plus.

Qualifications

• Unencumbered State of Illinois RN license required.
• Graduate of an accredited nursing program.
• Prior experience as a MDS Coordinator.
• 1 year RN experience in a Skilled Nursing Facility encouraged.
• CNA experience preferred.

Responsibilities

• Develop, monitor, audit, and modify each resident's care plan for individual needs and goals.
• Perform resident assessments and assist in the discharge process.

Education

Graduate of an accredited nursing program
