Job summary

The Grove of La Grange Park is seeking an RN MDS Coordinator to join our caring, team-oriented post-acute community. You will lead resident assessments, develop and monitor care plans, and collaborate with interdisciplinary teams to ensure quality outcomes.

Qualified candidates have an active Illinois RN license, a nursing program graduate, CPR certification, and prior MDS coordination in a skilled nursing setting. We value growth, supportive leadership, and a stable, home-like work environment.

Responsibilities

As a RN MDS Coordinator you are instrumental in giving your team the knowledge they need to care for each resident’s unique needs. Your work will ensure our residents receive the high standard of care they have grown to expect at The Grove La Grange Park by developing, monitoring, auditing, and modifying each resident’s care plan for their individual needs and goals, performing resident assessments and assisting in the discharge process. Our residents will depend on your knowledge, skills, and attention to detail to ensure they are comfortable and safe.

To be eligible for consideration applicants should have:

As a minimum, an unencumbered State of Illinois R.N. License, be a graduate of an accredited nursing program and C.P.R. Certification. Prior experience as a MDS Coordinator, one (1) year of experience as an R.N. in a Skilled Nursing Facility setting and (1) year of experience as a C.N.A. is preferable but we are willing to invest in the right candidate!
