Job summary

The Grove of La Grange Park is seeking an RN MDS Coordinator to join our caring team. You will develop, monitor and audit resident care plans, conduct assessments, and assist with discharge planning to ensure high-quality, person-centered care in a post-acute setting.

Ideal candidates will hold an Illinois RN license and have completed an accredited nursing program; prior MDS coordination experience is preferred and CNA experience is a plus.

Qualifications

• Unencumbered State of Illinois RN license required.
• Graduate of an accredited nursing program.
• Prior experience as a MDS Coordinator.
• 1 year RN experience in a Skilled Nursing Facility encouraged.
• CNA experience preferred.

Responsibilities

• Develop, monitor, audit, and modify each resident's care plan for individual needs and goals.
• Perform resident assessments and assist in the discharge process.

Education

Graduate of an accredited nursing program

As a RN MDS Coordinator you are instrumental in giving your team the knowledge they need to care for each resident's unique needs. Your work will ensure our residents receive the high standard of care they have grown to expect at The Grove of La Grange Park by developing, monitoring, auditing, and modifying each resident's care plan for their individual needs and goals, performing resident assessments and assisting in the discharge process. Our residents will depend on your knowledge, skills, and attention to detail to ensure they are comfortable and safe.

To be eligible for consideration applicants should have:

As a minimum, an unencumbered State of Illinois R.N. License, be a graduate of an accredited nursing program and C.P.R. Certification. Prior experience as a MDS Coordinator, one (1) year of experience as an R.N. in a Skilled Nursing Facility setting and (1) year of experience as a C.N.A. is preferable but we are willing to invest in the right candidate!
